Dance Camp Tap Shoe Rental Form

Please fill out the form below to rent tap shoes for your upcoming dance camp with Dance Co (note that we will need a submission for each week of camp that you are attending).

We will place the rented tap shoes in your studio for the first day of camp. Should the dancer need a size swap, we will assist them!

The full price of the tap shoe will be charged to your credit card on file if:

1) The tap shoes have been damaged or are missing any parts.

2) The tap shoes are not returned by the end of the camp you are attending.
